For 2026-2027 School Year
For all public middle school students, sign up is now located on your school registration form! Be sure to check YES on the question that asks if you'd like to receive a yes card. After that, no need to do anything- you will receive an email later in the summer about finishing up the process to get your card issued.

If you already registered your middle school child for next year and aren't sure if you checked the box.. that's ok! Fill out this request form to ensure your child gets a Yes Card next year.

All middle school youth in Franklin County are eligible for a Yes Card. If you have a 6th-8th grader who is homeschooled or attends private school, please fill out this request form.
